Abdominal scar endometriosis in a patient with ovarian endometrioma

Abstract

Endometriosis is defined as an ectopic implantation of uterine endometrial tissue outside the uterine cavity, usually in the pelvis. While the diagnosis of extrapelvic endometriosis is difficult due to its rarity and diverse symptoms, more than half of them develop in the abdominal wall. However, abdominal wall endometrioma is a rare condition, which usually develops on a surgical scar of Cesarean section or hysterectomy. A surgical team of both gynecologic and plastic surgeons operated together on a 44 year old patient who had a 7.0x6.5x4.5 cm abdominal wall endometrioma at her Cesarean section scar as well as an ovarian endometrioma with severe pelvic adhesion. The pelvic endometriosis was removed radically by total hysterectomy with bilateral salpingo-oophorectomy in order to reduce the possibility of recurrence. Abdominal wall endometrioma was excised conservatively to save the anterior abdominal fascia layer for the purpose of preventing complications. The wide abdominal wall defect was reconstructed by adopted mini-abdominoplasty technique. These procedures led us to a functionally and cosmetically satisfactory result. Thus, we present a successful treatment case of extensive Cesarean scar endometriosis with a brief review of literatures.
